IV Maintenance Fluid Rate for Adults

Calculation of Maintenance Fluid Requirements

  • The European Society for Clinical Nutrition and Metabolism (ESPEN) recommends at least 1.6 L of fluids daily for adult women, which translates to approximately 1,530 mL per 24 hours for a 42.5kg female patient, using the 1.5 mL/kg/hour maintenance rate 1
  • The ESPEN geriatrics guidelines specify that older women should receive at least 1.6 L of drinks daily, while men require at least 2.0 L daily, unless clinical conditions dictate otherwise 1

Fluid Type Selection

  • The Infectious Diseases Society of America (IDSA) emphasizes isotonic crystalloids, such as 0.9% normal saline or lactated Ringer's solution, for IV hydration when oral intake is inadequate 2, 3
  • Isotonic crystalloid solutions, such as 0.9% normal saline or lactated Ringer's solution, are appropriate first-line choices for maintenance fluid 2, 3, 4

Clinical Context and Adjustments

  • The maintenance fluid rate assumes euvolemia at baseline, and initial resuscitation with boluses may be needed before transitioning to maintenance rates if the patient has any degree of dehydration 4
  • Patients with heart failure or renal failure may require fluid restriction below standard maintenance rates 1
  • Monitoring parameters, such as hydration status, daily weights, and electrolyte monitoring, are essential to guide fluid composition adjustments and detect potential complications 4, 6

Practical Implementation

  • The maintenance IV fluid rate for a 42.5kg patient should be ordered at 60-65 mL/hour, using 0.9% normal saline or lactated Ringer's solution, and reassessed every 8-12 hours for signs of adequate hydration or fluid overload 2, 4
