Management of Post-Operative Headache

Initial Assessment

  • Evaluate patients with glioblastoma multiforme for concerning features that might indicate complications, including new focal neurological deficits, signs of increased intracranial pressure, and fever or other signs of infection, as recommended by the British Journal of Cancer and Praxis Medical Insights 2 3

First-Line Treatment

  • Implement a multimodal analgesic strategy starting with non-opioid medications, including acetaminophen, magnesium sulfate, ondansetron, and saline bolus, as recommended by Praxis Medical Insights 1
  • The American College of Cardiology is not mentioned in this article, however, Praxis Medical Insights recommends a migraine cocktail consisting of saline bolus, ondansetron, magnesium, and acetaminophen as the most effective first-line treatment for post-operative headache in patients with glioblastoma multiforme 1

Second-Line Treatment Options

  • Consider dexamethasone to reduce cerebral edema, which is particularly important in post-GBM surgery patients, as recommended by Praxis Medical Insights 1

Special Considerations for GBM Patients

  • Monitor for potential complications that may cause or worsen headache, including post-operative cerebral edema, cerebrospinal fluid leak, and intracranial hypotension, as recommended by British Journal of Cancer and Praxis Medical Insights 3 4
  • Consider imaging if headache pattern changes or worsens, as this may indicate tumor recurrence or post-surgical complications, as recommended by World of Neurosurgery 5

Prevention Strategies

  • Continue prescribed corticosteroid taper as directed to manage cerebral edema, as recommended by British Journal of Cancer 4
  • For patients with previous seizures, maintain anticonvulsant treatment as seizures can trigger or worsen headaches, as recommended by British Journal of Cancer 2

Important Pitfalls to Avoid

  • Avoid abrupt discontinuation of corticosteroids as this can worsen headache and cerebral edema, as recommended by British Journal of Cancer 4
  • Be cautious with opioids due to risk of respiratory depression, especially in patients with potential increased intracranial pressure, as recommended by Praxis Medical Insights 1
