Depression and Neuropsychiatric Risks of 5α‑Reductase Inhibition

Evidence of Depression Risk

  • DHT inhibition with 5α‑reductase inhibitors (finasteride, dutasteride) is linked to a modest but measurable increase in depression risk; however, the data supporting persistent neurological disease or long‑term mood disorders after drug discontinuation are of low quality and remain controversial. 1

Guideline Classifications

  • The 2025 European Urology guideline classifies 5α‑reductase inhibitors as drug‑induced causes of hypogonadism and notes their systemic effects beyond the prostate and scalp, but it does not require routine psychiatric monitoring. 3
  • No major clinical guideline (including the 2009 ASCO/AUA guideline, the 2009 Journal of Urology guideline, and the 2010 National Comprehensive Cancer Network guideline) recommends routine psychiatric monitoring for patients receiving 5α‑reductase inhibitors. 1, 3, 6
  • The 2009 ASCO/AUA guideline specifically states that sexual side effects are “reversible and therefore uncommon after the first year of therapy” and makes no mention of psychiatric surveillance. 1

Practical Monitoring Recommendations

  • Before initiating a 5α‑reductase inhibitor, clinicians should screen for baseline mood symptoms (e.g., brief questionnaire), but a formal psychiatric evaluation is not required. 3

Discontinuation and Adverse‑Event Profile

  • In randomized trials, overall discontinuation rates for finasteride are comparable to placebo (≈ 15 %). Discontinuation specifically due to adverse events occurs in 6–7 % of participants in both groups, indicating that adverse‑event–driven dropout is not higher with the drug. 1

Herbal Supplements (Saw Palmetto)

  • Saw palmetto and similar herbal products may contain phytoestrogenic compounds that can influence hormone levels, yet the composition is poorly characterized and the evidence for any psychiatric effects is extremely limited. 6
