FDA Pregnancy Category Classification

Understanding Pregnancy Categories

  • The FDA pregnancy classification system includes categories such as Category A, where controlled studies show no risk to fetus, Category B, where animal studies show no risk but no adequate human studies, Category C, where animal studies show adverse effects with no adequate human studies, Category D, where evidence of human fetal risk exists but benefits may outweigh risks, and Category X, where the drug is contraindicated due to proven fetal abnormalities 1, 2, 3, 4
  • Category A is defined as controlled studies showing no risk to fetus, according to the Journal of Allergy and Clinical Immunology 4
  • Category B is defined as animal studies showing no risk, but no adequate human studies, or animal studies showing adverse effects not confirmed in human studies, as stated by the Journal of Allergy and Clinical Immunology 4
  • Category C is defined as animal studies showing adverse effects with no adequate human studies, or no studies available in animals or humans, as reported by the Journal of Allergy and Clinical Immunology 4
  • Category D is defined as evidence of human fetal risk, but benefits may outweigh risks in certain situations, according to the European Heart Journal 1
  • Category X is defined as contraindicated due to proven fetal abnormalities, as stated by the European Heart Journal 1

Clinical Recommendations

  • The European Heart Journal recommends that levodropropizine should be avoided during pregnancy unless the clinical situation is urgent and no safer alternatives exist 1, 2
  • A risk-benefit assessment is required when drug treatment is necessary during pregnancy, weighing the potential risk against the possible benefit to the mother, as recommended by the European Heart Journal 1, 2, 3
  • The Journal of Allergy and Clinical Immunology suggests considering medications with established pregnancy safety profiles before using levodropropizine 4
