Albuterol Inhaler Dosing Guidelines for Asthma and COPD Management

Standard Dosing Guidelines

  • The American College of Allergy, Asthma, and Immunology recommends using albuterol as needed at a dose of 2 puffs (90 mcg/puff) every 4-6 hours, not exceeding 8 puffs per day for standard maintenance therapy of asthma or COPD 1
  • For adults, the recommended dose is 2 puffs (180-200 mcg) every 4-6 hours as needed via metered-dose inhaler (MDI) 1, 2
  • For children 5-11 years, the recommended dose is 2 puffs every 4-6 hours as needed via MDI 1, 3

Special Situations

  • For exercise-induced bronchospasm, 2 puffs of albuterol should be administered 15-30 minutes before exercise 1, 2
  • For mild-to-moderate acute exacerbations, 4-8 puffs via MDI with spacer every 20 minutes for 3 doses, then every 1-4 hours as needed is recommended 3

Monitoring and Safety Considerations

  • Common side effects of albuterol include tachycardia, skeletal muscle tremor, hypokalemia, headache, and hyperglycemia 1, 2
  • Elderly patients should use albuterol with caution due to increased risk of tremor and potential cardiac effects 5

Alternative Options

  • Levalbuterol (R-albuterol) can be used at half the milligram dose of albuterol with comparable efficacy and safety 3, 2

Important Clinical Pearls

  • Albuterol is not recommended for long-term daily treatment; it should be used as a rescue medication only 1, 2
  • For patients requiring albuterol more than twice weekly, initiation or adjustment of controller medications should be considered 2
