Hydralazine Dosing for ICH Blood Pressure Control

Introduction to Hydralazine Limitations

  • The American College of Cardiology and American Heart Association guidelines state that hydralazine's unpredictability of response and prolonged duration of action make it less desirable for acute ICH management 1, 2
  • The American College of Cardiology recommends nicardipine as the preferred agent for ICH blood pressure control, starting at 5 mg/h IV and increasing by 2.5 mg/h every 5 minutes to a maximum of 15 mg/h, due to its precise and titratable control 1, 2
  • The American College of Cardiology also recommends labetalol as an acceptable alternative, with a dose of 0.3-1.0 mg/kg slow IV injection every 10 minutes or 0.4-1.0 mg/kg/h IV infusion up to 3 mg/kg/h 1, 2

Critical ICH Blood Pressure Targets

  • The American Heart Association recommends targeting a systolic blood pressure of 140-160 mmHg within 6 hours of symptom onset to prevent hematoma expansion 3, 4
  • The American Heart Association also recommends avoiding excessive drops in blood pressure (>70 mmHg within 1 hour) to reduce the risk of acute kidney injury and mortality 3, 4
  • Maintaining a cerebral perfusion pressure of ≥60 mmHg at all times is recommended 3, 4
  • A mean arterial pressure of <130 mmHg is recommended 3
