Treatment of Microangiopathic Hemolytic Anemia (MAHA)
Diagnostic Approach
- The American Society of Clinical Oncology recommends obtaining a blood smear to identify schistocytes, which are characteristic of MAHA, and checking laboratory values including complete blood count, LDH, haptoglobin, reticulocyte count, and bilirubin in patients with suspected MAHA 1
- The American Society of Clinical Oncology suggests testing ADAMTS13 activity and inhibitor levels to distinguish TTP from other forms of thrombotic microangiopathy (TMA) 3
- Evaluating renal function helps differentiate between various forms of TMA, according to the American Society of Clinical Oncology 1
- Ruling out other causes of hemolysis through direct antiglobulin test (Coombs test) is recommended by the American Society of Clinical Oncology 1
Treatment Algorithm Based on Etiology
- The American College of Physicians recommends initiating plasma exchange immediately in suspected TTP (ADAMTS13 activity <10%), as this is life-saving and should not be delayed while awaiting ADAMTS13 results 1
- Administering high-dose corticosteroids (methylprednisolone 1 g IV daily for 3 days) is suggested by the American Society of Clinical Oncology for TTP treatment 1
- The American Society of Clinical Oncology recommends adding rituximab early in the treatment course to reduce relapse risk in TTP patients 1
- For complement-mediated atypical HUS, the American Society of Nephrology suggests considering eculizumab therapy (900 mg weekly for four doses, then 1,200 mg week 5, followed by 1,200 mg every 2 weeks) 4
Supportive Care
- The American Society of Clinical Oncology recommends RBC transfusions according to existing guidelines, transfusing only the minimum necessary to relieve symptoms or return to safe Hb range (7-8 g/dL) 1
- Folic acid supplementation (1 mg daily) is suggested by the American Society of Clinical Oncology for patients with MAHA 1
- Monitoring hemoglobin levels weekly until treatment is complete is recommended by the American Society of Clinical Oncology 1
Special Considerations
- The American College of Rheumatology recommends continuing hydroxychloroquine if applicable and starting low-dose aspirin before 16 weeks gestation in patients with a history of TMA 3
- The American Academy of Pediatrics suggests that the treatment approach for pediatric patients is similar to adults but requires dose adjustments and consideration of growth, fertility, and psychosocial factors 3
- The American Geriatrics Society recommends considering comorbidities when selecting therapy, particularly with immunosuppressive agents, in elderly patients 5
Monitoring and Follow-up
- Regular blood counts to detect worsening cytopenias are recommended by the European Society for Medical Oncology 6
- Monitoring for iron overload in patients receiving transfusions is suggested by the European Society for Medical Oncology 6
- The American Society of Clinical Oncology recommends maintaining a high index of suspicion for TTP, as delay in diagnosis and treatment significantly increases mortality 1
