Cervical Spondylosis Management
Pathophysiology and Clinical Presentation
- Radiculopathy and myelopathy can occur due to degenerative changes in the cervical spine, causing arm pain, neurological symptoms, and dysfunction of the spinal cord, with decreased range of motion in the neck, according to the Journal of Neurosurgery 1
- Radicular symptoms may include pain radiating down the arm, numbness or tingling in the arms or hands, and weakness in specific muscle groups, as reported in the Journal of Neurosurgery 2
- Myelopathic symptoms may include generalized weakness or stiffness in the legs, as noted in the Journal of Neurosurgery 1
Natural History and Prognosis
- The natural history of cervical spondylosis is variable, with many patients experiencing long periods of stable symptoms or slow progression, and approximately 70% of patients with mild cervical spondylotic myelopathy maintaining their clinical status over 3 years with nonoperative treatment, as stated in the Journal of Neurosurgery 1
- Younger patients and those with milder disability have better prognosis, according to the Journal of Neurosurgery 3
Treatment Options
- Anti-inflammatory medications may be used for mild symptoms without significant neurological deficits, as reported in the Journal of Neurosurgery 4
- Surgical approaches, including anterior and posterior approaches, may be effective for progressive neurological deficits, severe or persistent pain, and moderate to severe myelopathy, with good to excellent outcomes in approximately 90% of patients with radiculopathy treated with ACDF, as noted in the Journal of Neurosurgery 2 and Praxis Medical Insights 5
Treatment for Cervical Spondylosis
Initial Management
- For a 38-year-old female with multilevel cervical spondylosis and disc space narrowing at C5-C6 and C6-C7 with normal alignment, initial management should focus on conservative treatment including neck immobilization, anti-inflammatory medications, and activity modification for at least 3 months before considering surgical options 6
