Drug Interaction Between Celecoxib and Cabozantinib

Introduction to Risks

  • The American Academy of Family Physicians recommends avoiding the combination of celecoxib and cabozantinib due to increased bleeding risk and nephrotoxicity, particularly in patients with bleeding disorders or impaired renal function 1

Mechanism of Interaction

  • The combination of celecoxib and cabozantinib increases bleeding risk through additive effects on platelet function and vascular integrity, and compounds nephrotoxicity through independent mechanisms of renal dysfunction 1, 2

Critical Risk in Bleeding Disorders

  • Patients with bleeding disorders or history of bleeding should not receive this combination, as NSAIDs increase bleeding risk three to six times when combined with anticoagulants, and the risk of GI bleeding recurrence is 5% in the first six months 1

Severe Nephrotoxicity Risk in Renal Impairment

  • The American Heart Association suggests that NSAIDs should be avoided entirely in persons with renal disease, as approximately 2% of patients discontinue NSAIDs due to renal complications, and celecoxib blocks prostaglandin-mediated renal vasodilation 1, 2

Specific Monitoring Protocol If Combination Cannot Be Avoided

  • The American College of Cardiology recommends implementing an intensive monitoring protocol, including baseline assessment and weekly monitoring for the first 3 weeks, if the combination cannot be avoided 3

Safer Alternative Strategies

  • Acetaminophen is the preferred first-line analgesic for patients requiring cabozantinib who need pain management, as it avoids both bleeding and renal risks associated with NSAIDs 3

Risk Stratification for Decision-Making

  • Absolute contraindications to combination include active bleeding disorder or thrombocytopenia, GFR <30 mL/min/1.73 m², history of NSAID-associated bleeding, cirrhosis with ascites, and congestive heart failure 1, 2, 3, 4, 5
