Management of Tympanic Membrane Removal

Critical First Principle: Avoid All Ototoxic Preparations

  • The American Academy of Otolaryngology-Head and Neck Surgery recommends avoiding aminoglycoside-containing drops, as they cause severe permanent sensorineural hearing loss after exposure through perforations 1
  • Fluoroquinolone eardrops, such as ofloxacin or ciprofloxacin-dexamethasone, are the only safe topical antibiotics for patients with tympanic membrane removal or perforation, due to their lack of ototoxicity even with direct middle ear exposure 1
  • Alcohol-containing preparations should be avoided, as alcohol in the middle ear space is both painful and potentially ototoxic 1

Immediate Protective Measures

  • Keeping the ear dry is crucial to prevent infection, and patients should use ear plugs or cotton balls coated with petroleum jelly when showering, and avoid swimming until the perforation has healed 1

Topical Antibiotic Therapy

  • Ciprofloxacin-dexamethasone combination drops are first-line therapy for topical antibiotic treatment, due to their superior clinical efficacy and cost-effectiveness 1
  • Topical therapy delivers drug concentrations 100-1000 times higher than systemic antibiotics can achieve at the infection site 1

Special Considerations

  • The American Academy of Otolaryngology-Head and Neck Surgery recommends exercising extreme caution with any ear manipulation in diabetic patients, due to their higher risk of malignant otitis externa 2
  • Diabetic patients may benefit from reacidifying the ear canal with vinegar or acetic acid drops after treatment, and may require a lower threshold for systemic antibiotics 2

Surgical Referral Indications

  • Patients should be referred to otolaryngology when repeated attempts at management are unsuccessful, complications are encountered, or specialized equipment is needed 3
