Treatment of Periodic Lateralized Epileptiform Discharges (PLEDs)

Efficacy of Levetiracetam in Treating Seizures and PLEDs

  • Levetiracetam has been shown to be effective in treating seizures and PLEDs, with a similar efficacy to valproate in treating benzodiazepine-resistant status epilepticus, with a response rate of 73% in patients with status epilepticus 1, 2, 3
  • A retrospective study found that levetiracetam had a response rate of 67% in patients with status epilepticus who received a dose of 20 mg/kg IV 2, 3, 4
  • A prospective series in elderly patients with documented status epilepticus on EEG found that 89% of patients showed a reduction in seizures and 78% experienced complete cessation of seizures after receiving levetiracetam 1,500 mg 2, 3, 4

Dosage and Administration

  • The recommended dose of levetiracetam is 30 mg/kg IV at a rate of 5 mg/kg per minute 5
  • It is recommended to consider a higher dose (30 mg/kg) in benzodiazepine-resistant status epilepticus 1, 2, 3

Important Considerations

  • It is essential to monitor the clinical response and EEG after administering the medication 2, 3, 4
