Oscillopsia Following Labyrinthitis

Clinical Presentation and Diagnosis

  • Patients recovering from labyrinthitis may experience visual blurring during head movements, difficulty changing focus from near to far, eye strain and fatigue, and vision-derived nausea, according to the American Academy of Otolaryngology-Head and Neck Surgery 1
  • Labyrinthitis is distinguished from other conditions by causing sudden severe vertigo with profound hearing loss lasting more than 24 hours, which is not episodic or fluctuating, as noted by the American Academy of Otolaryngology-Head and Neck Surgery 2
  • This condition can be differentiated from Ménière's disease, which has episodic and fluctuating symptoms, and from vestibular neuritis, which presents with vertigo without hearing loss, as stated by the American Academy of Otolaryngology-Head and Neck Surgery 2
  • Benign paroxysmal positional vertigo (BPPV) can also be ruled out, as it is characterized by brief positional vertigo lasting seconds, according to the American Academy of Otolaryngology-Head and Neck Surgery 2

Prognosis and Management

  • When oscillopsia or visual symptoms accompany vertigo after presumed labyrinthitis, it is essential to carefully assess for central causes, particularly if there are additional neurological signs, which may indicate stroke or other central pathology rather than isolated labyrinthitis, as advised by the American Academy of Otolaryngology-Head and Neck Surgery 4
