Osteoporosis Screening Guidelines for High-Risk Patients

Risk Factor Assessment

  • A history of hysterectomy, particularly at a young age, can lead to premature estrogen deficiency and increased risk of osteoporosis, as noted by the Cancer Treatment Reviews 3
  • Hysterectomy has been associated with earlier onset of menopausal symptoms and potential impacts on bone health, according to the Cancer Treatment Reviews 3

Screening Recommendations Based on Guidelines

  • The US Preventive Services Task Force recommends selective screening with DEXA in postmenopausal women younger than 65 years who have an elevated risk of osteoporosis based on a formal clinical risk assessment tool, as stated in JAMA 1, 2
  • The American College of Obstetricians and Gynecologists recommends selective BMD screening with DEXA in women younger than 65 years who have elevated risk of osteoporosis based on clinical risk assessment, as reported in JAMA 2
  • The American Association of Clinical Endocrinologists recommends evaluating all women 50 years or older for fracture risk and considering BMD measurement based on clinical fracture risk profile, as noted in JAMA 2

Diagnostic Approach

  • A DEXA scan of the lumbar spine and hip is the primary diagnostic modality for assessing bone density, according to the Journal of the American College of Radiology 5, 6
  • Vertebral fracture assessment (VFA) should be considered if the initial T-score is less than -1.0, as recommended by the Journal of the American College of Radiology 6
  • The results of the DEXA scan should be used to calculate 10-year fracture risk using FRAX or another validated tool, as stated in the European Journal of Nuclear Medicine and Molecular Imaging 4, 7

Follow-up and Monitoring

  • Follow-up DEXA scans should be performed every 2 years if the initial scan shows osteopenia or osteoporosis, according to the Journal of Clinical Oncology 8
