Switching from Zoledronate to Denosumab

Rationale for Switching

  • Both zoledronate and denosumab are effective bone-targeting agents recommended for patients with bone metastases, according to the European Society for Medical Oncology 1
  • Switching between these agents may be considered throughout the course of disease management, as stated by the European Society for Medical Oncology 1
  • Denosumab may be preferred in patients with renal impairment, as suggested by the European Society for Medical Oncology 1

Important Considerations When Switching

  • Denosumab should be administered every 4 weeks for treatment of bone metastases, as recommended by the European Society for Medical Oncology 1
  • The National Comprehensive Cancer Network recommends calcium supplementation and vitamin D3 supplementation during denosumab therapy 3
  • Correction of any vitamin D deficiency before starting denosumab is essential, according to the European Society for Medical Oncology 2

Critical Warning: Rebound Effect After Discontinuation

  • If denosumab is discontinued for more than 6 months, bisphosphonate treatment is recommended to suppress rebound osteolysis, as stated by the European Society for Medical Oncology 1, 4
  • Denosumab discontinuation without follow-up therapy can lead to rapid bone loss and increased fracture risk, according to the European Society for Medical Oncology 2
