Management of Post-Operative Endophthalmitis and Retinal Detachment

Introduction to Endophthalmitis Management

  • The American Academy of Ophthalmology recommends intravitreal antibiotics, including vancomycin and ceftazidime, as the standard of care for endophthalmitis management to reduce inflammation and provide comprehensive antimicrobial treatment 1
  • The combination of vancomycin and ceftazidime is the standard empiric therapy for post-operative endophthalmitis, providing gram-positive and gram-negative coverage 1

Diagnostic Procedures

  • Optical Coherence Tomography (OCT) is medically necessary to evaluate macular status post-vitrectomy, assess for macular edema, monitor retinal attachment status, and evaluate treatment response in patients with a history of retinal detachment 2
  • Fundus Photography is medically necessary to document retinal status, track disease progression, and compare with previous images to evaluate treatment effectiveness in patients with a history of retinal detachment 2

Follow-Up and Monitoring

  • Regular follow-up is essential for endophthalmitis management, including interval history of new symptoms, visual acuity measurement, intraocular pressure monitoring, slit-lamp biomicroscopy of anterior chamber, and indirect ophthalmoscopy of retina 2
  • The American Academy of Ophthalmology recommends close monitoring of patients with endophthalmitis to prevent complications such as permanent vision loss, retinal detachment, and progression to phthisis bulbi 1
  • Patients should be evaluated on the first post-operative day, at least one visit during the first 1-2 weeks, and additional visits during a 3-month period to assess visual acuity, IOP, and status of the anterior segment, with more frequent follow-up for patients with complications 3, 4

Risk of Complications

  • Endophthalmitis is a vision-threatening emergency that requires prompt diagnosis with appropriate imaging, aggressive treatment with intravitreal antibiotics, and close follow-up to monitor response, with a risk of retinal detachment occurring in approximately 10% of cases 1
  • Patients who have undergone retinal detachment surgery are at risk for post-operative complications, including elevated or low intraocular pressure, retinal re-detachment, endophthalmitis, and gas-related complications, which can present with symptoms such as headache, eye pain, and dizziness 3
  • Patients should be educated on the importance of reporting symptoms such as new or worsening floaters, visual field changes, decreased visual acuity, pain, and redness or discharge, as these may indicate serious complications following retinal detachment surgery 3
  • Emphasizing the importance of maintaining head position if intraocular gas was used during surgery is crucial to prevent gas-related complications in patients who have undergone retinal detachment surgery 3, 5

Post-Operative Care

  • NSAIDs, such as ibuprofen, can be effective for post-operative pain control with fewer side effects than opioids in patients who have undergone retinal detachment surgery, but patients should be monitored for potential NSAID side effects, including gastrointestinal issues 3
  • The American Academy of Ophthalmology guidelines recommend urgent ophthalmologic examination, including interval history, measurement of intraocular pressure, slit-lamp biomicroscopy of the anterior segment, examination of wound sites, indirect binocular ophthalmoscopy of the peripheral retina, and assessment for retinal breaks or new detachment, if patients experience symptoms such as headaches, positional dizziness, and eye pain following retinal detachment surgery 5, 3
  • Patients should be instructed to notify their ophthalmologist promptly if they experience new symptoms or change in symptoms, pain, worsening vision, or other concerns of retinal complications, and the operating ophthalmologist should be contacted immediately to arrange for same-day evaluation 3, 5
