Lasilactone Dosing for Heart Failure with Reduced Ejection Fraction

Introduction to Lasilactone Dosing

  • The European Society of Cardiology recommends an initial dosage of Lasilactone (spironolactone/hydrochlorothiazide) of 1/2 tablet (10mg/25mg) daily for patients with heart failure and an ejection fraction of 37%, with potential titration to 1 tablet (20mg/50mg) daily if tolerated and clinically indicated 1, 2

Rationale for Dosing Recommendation

  • An ejection fraction of 37% falls into the reduced ejection fraction category (<50%), requiring specific heart failure management strategies, including the use of mineralocorticoid receptor antagonists (MRAs) like spironolactone, as recommended by the European Society of Cardiology 2
  • For patients with reduced EF (<50%), low-dose loop diuretics and MRAs like spironolactone should be considered to improve symptoms and reduce hospitalization risk, according to the European Society of Cardiology 1, 2

Dosing Strategy

  • Monitor renal function and electrolytes (particularly potassium) at 1-2 weeks after initiation, and blood pressure and clinical status (symptoms, signs of congestion, body weight), as recommended by the European Journal of Heart Failure 3
  • If well tolerated after 2-4 weeks with stable renal function and potassium levels, consider uptitration to 1 tablet daily, based on guidelines from the European Journal of Heart Failure 3

Special Considerations

  • Patients with heart failure and reduced EF should receive comprehensive therapy including an ACE inhibitor or ARB, a beta-blocker, and an MRA (spironolactone component of Lasilactone), as recommended by the European Society of Cardiology 1, 2

Monitoring Requirements

  • Check renal function and electrolytes 1-2 weeks after initiation, 1-2 weeks after each dose increase, and every 3-6 months during maintenance therapy, according to the European Journal of Heart Failure 3
  • More frequent monitoring is required in patients with a history of renal dysfunction or electrolyte abnormalities, as recommended by the European Journal of Heart Failure 3

Efficacy Considerations

  • The diuretic component of Lasilactone helps manage fluid retention and congestion symptoms, as recommended by the European Society of Cardiology 1, 2

Dose Adjustment Algorithm

  • If symptoms of congestion worsen, consider temporary increase in hydrochlorothiazide component or addition of loop diuretic, based on guidelines from the European Journal of Heart Failure 3, 4
