Management of Constipation in Patients with Tumor Lysis Syndrome

Indications for Renal Replacement Therapy

  • Indications for renal replacement therapy in patients with tumor lysis syndrome include persistent hyperkalemia, severe metabolic acidosis, volume overload unresponsive to diuretic therapy, and overt uremic symptoms, as recommended by the Haematologica guideline 1, 3, 4
  • The European Society of Haematology recommends renal replacement therapy for patients with severe metabolic acidosis and volume overload unresponsive to diuretic therapy 2

Management Approach for Constipation

  • For patients with tumor lysis syndrome and renal impairment requiring constipation management, senna is preferred over Miralax as it works through local action in the colon and has minimal systemic absorption, according to the American Society of Clinical Oncology 2
  • The National Comprehensive Cancer Network recommends monitoring electrolyte levels closely, as both tumor lysis syndrome and certain laxatives can affect electrolyte balance 2

Special Considerations

  • In patients with severe renal impairment or uremic encephalopathy, renal replacement therapy should be initiated promptly, as recommended by the Kidney Disease: Improving Global Outcomes guideline 2
  • The American College of Nephrology recommends continuous renal replacement therapy over intermittent hemodialysis for hemodynamically unstable patients 2
