Treatment for Stage 3 Liver Fibrosis Based on Mean Liver Stiffness
Etiology-Specific Treatment Approach
- The American Association for the Study of Liver Diseases recommends resmetirom as the first FDA-approved pharmacologic therapy specifically for F2-F3 fibrosis in MASLD/MASH patients, with treatment eligibility criteria including liver stiffness by VCTE 10-15 kPa, platelet count >140,000/μL, and no evidence of portal hypertension 1, 2
- For MASLD/MASH-related F3 fibrosis, treatment eligibility criteria also include phosphatidylethanol (PeTH) <200 to exclude significant alcohol use, and patients with cirrhosis indicators such as VCTE >20 kPa, MRE >5 kPa, or ELF >11.3 should be excluded from resmetirom therapy 1, 2
- Lifestyle modifications, including a target 7-10% body weight loss through diet and exercise, and aggressive management of metabolic comorbidities, remain foundational for MASLD/MASH patients 5, 6
Mandatory Surveillance for All F3 Patients
- The American Association for the Study of Liver Diseases recommends HCC surveillance every 6 months for all patients with F3 fibrosis, regardless of treatment or etiology, using abdominal ultrasound and considering alpha-fetoprotein as an adjunct but not a replacement for imaging 3, 4, 7
- Surveillance should continue indefinitely even after successful treatment, as HCC risk persists though reduced 4, 7
Portal Hypertension Assessment
- All F3 patients require evaluation for portal hypertension, including screening for esophageal varices with upper endoscopy, assessing for clinical signs such as splenomegaly and thrombocytopenia, and considering the presence of portal hypertension in treatment decisions 1, 2, 3, 4
Monitoring Disease Progression
- Serial non-invasive testing, including liver stiffness measurement and FIB-4 calculation, should be used to monitor fibrosis progression or regression in F3 patients, but non-invasive tests should not be used to assess fibrosis stage after successful antiviral therapy for hepatitis C 3, 5, 8
