Recommended Duration of Cefazolin Prophylaxis Post-Surgery

General Principles for Cefazolin Prophylaxis

  • For most surgical procedures, a single preoperative dose of cefazolin is sufficient and post-operative continuation is not necessary, as recommended by the American Society of Anesthesiologists 1, 2, 3
  • Re-dosing during surgery is only indicated if the procedure duration exceeds 4 hours (for cefazolin 2g) or 2 hours (for cefazolin 1.5g), according to the American College of Surgeons 1, 4

Specific Post-Surgical Duration Guidelines

  • Standard procedures: A single dose of cefazolin only, with no post-operative continuation, is recommended by the American College of Surgeons 1, 2, 5
  • Cranio-cerebral wounds: Cefazolin prophylaxis should be continued for up to 48 hours maximum, as suggested by the American Association of Neurological Surgeons 6, 7
  • Limb amputation: Cefazolin prophylaxis should be continued for 48 hours post-surgery, according to the American College of Surgeons 8, 9

Dosing Considerations

  • Bariatric surgery: A dose of 4g IV (30-minute infusion) with re-injection of 2g if duration exceeds 4 hours is recommended by the American Society for Metabolic and Bariatric Surgery 1, 5
