Cardiac Risks Associated with Donepezil

Primary Cardiac Risks

  • Case reports describe paroxysmal atrial fibrillation with acetylcholinesterase inhibitors including donepezil, potentially related to increased vagal tone causing heterogeneous shortening of atrial action potential duration and effective refractory period, in patients with pre-existing heart conditions 1
  • Myocardial ischemia secondary to coronary vasospasm may also trigger atrial fibrillation with parasympathomimetic drugs, such as donepezil, in patients with cardiovascular risk factors 1

Clinical Management Algorithm

  • The American Geriatrics Society recommends obtaining baseline ECG to identify pre-existing conduction abnormalities, particularly PR interval and heart rate, and documenting all concurrent medications, especially beta-blockers, calcium channel blockers, or other negatively chronotropic/dromotropic agents, before initiating donepezil therapy 2, 3
  • The American Geriatrics Society suggests assessing for history of syncope, bradycardia, heart block, or congestive heart failure, and exercising caution in patients already taking beta-blockers or other rate-controlling medications, as the combination of metoprolol and donepezil can cause additive bradycardia and fatigue 2, 3
  • Patients with congestive heart failure should be monitored closely, as cholinergic effects of donepezil may worsen cardiac function, according to the American Geriatrics Society 3
  • Elderly patients with multiple cardiovascular risk factors or established coronary artery disease should be monitored closely, as donepezil may increase cardiac risk, according to Pharmacological Research 4

Monitoring Strategy

  • Monitoring for symptoms of bradycardia, such as fatigue, dizziness, presyncope, or syncope, is recommended, particularly in patients with risk factors or those who develop symptoms, as donepezil may cause bradycardia, according to Pharmacological Research 1

Important Caveats

  • The American Geriatrics Society recommends weighing the modest cognitive benefits of donepezil against cardiac risks, particularly in patients with limited life expectancy and multiple comorbidities 3
