Nitrofurantoin Formulations for Uncomplicated UTIs

Introduction to Nitrofurantoin Formulations

  • The American Urological Association and other guideline societies recommend nitrofurantoin monohydrate/macrocrystals 100 mg twice daily for 5 days as first-line therapy for uncomplicated UTIs, due to its dual-release formulation and less frequent dosing 1, 2, 3

Formulation Differences and Efficacy

  • Macrobid (nitrofurantoin monohydrate/macrocrystals) is dosed as 100 mg twice daily for 5-7 days for uncomplicated UTIs, with clinical cure rates ranging from 88-93% and bacterial cure rates ranging from 81-92% 1, 2, 3
  • The monohydrate/macrocrystal formulation allows for less frequent dosing due to sustained release properties, improving compliance compared to four-times-daily Macrodantin 3
  • The IDSA/ESCMID guidelines recommend nitrofurantoin monohydrate/macrocrystals 100 mg twice daily for 5 days as first-line therapy for uncomplicated UTIs, with the AUA/CUA/SUFU guidelines listing nitrofurantoin as first-line therapy for recurrent UTIs 1, 2
  • For enterococcal UTIs, higher frequency dosing is required: 100 mg four times daily (every 6 hours) 4

Contraindications and Side Effects

  • Nitrofurantoin is contraindicated when creatinine clearance <60 mL/min, and in the last trimester of pregnancy, with serious side effects (pulmonary reactions, polyneuropathy) mainly occurring with long-term use 4, 5
  • Common side effects include nausea and headache, with adverse event rates ranging from 5.6-34% across studies 3, 4

Practical Clinical Recommendations

  • The IDSA/ESCMID guidelines specifically recommend the Macrobid formulation, with equivalent efficacy to trimethoprim-sulfamethoxazole and ciprofloxacin, and minimal collateral damage and resistance development 1, 2, 3
