Schizophrenia Spectrum Disorder Diagnostic Domains

Core Symptom Domains

  • The American Psychiatric Association defines schizophrenia spectrum disorders by five core symptom domains: delusions, hallucinations, disorganized thinking, disorganized or abnormal behavior, and negative symptoms, according to DSM-5 and ICD-11 diagnostic frameworks 1
  • Disorganized thinking, manifesting as loose associations, illogical thinking, and impaired discourse abilities, is a key domain in schizophrenia spectrum disorders 2
  • Negative symptoms encompass five specific domains: avolition, anhedonia, asociality, blunted affect, and alogia, as described in Psychiatry Research 3, 4

Dimensional Assessment Framework

  • The World Health Organization's ICD-11 uses dimensional symptom specifiers to describe current severity across six domains, including positive symptoms, negative symptoms, depressive symptoms, manic symptoms, psychomotor symptoms, and cognitive symptoms, with each domain rated on a 4-point scale from "not present" to "present and severe" 1

Clinical Significance of Domains

  • Cognitive symptoms affect approximately 80% of patients and contribute substantially to illness burden, as reported in the American Journal of Psychiatry 5, 6
