Thiamine Deficiency Treatment Guidelines
Diagnosis and Assessment
- The American College of Nutrition recommends suspecting thiamine deficiency in patients with risk factors including malnutrition, prolonged vomiting, poor dietary intake, high alcohol intake, rapid weight loss, bariatric surgery, or inability to tolerate vitamin supplements 2, 3
- Laboratory confirmation can be obtained by measuring red blood cell or whole blood thiamine diphosphate (ThDP), which is more reliable than plasma levels 1
- A therapeutic trial of thiamine is appropriate when deficiency is suspected, as treatment should not be delayed while awaiting laboratory results 1
Treatment Protocol
- For mild deficiency, the American College of Nutrition recommends starting with 10 mg/day oral thiamine for one week, followed by 3-5 mg daily for at least 6 weeks 1
- For patients on chronic diuretic therapy, the American College of Nutrition recommends providing 50 mg oral thiamine daily 1
- For moderate risk or deficiency, the American College of Nutrition recommends 100 mg IV three times daily 1
- For severe deficiency with neurological symptoms, the American College of Nutrition recommends 500 mg IV three times daily 1
- After IV treatment, the American College of Nutrition recommends transitioning to an oral maintenance dose of 50-100 mg/day 1
Special Considerations
- For refeeding syndrome, the American College of Nutrition recommends administering 300 mg IV thiamine before initiating nutrition therapy, followed by 200-300 mg IV daily for at least 3 more days 1
- For critically ill patients, the American College of Nutrition recommends 100-300 mg/day 1
- For patients undergoing continuous renal replacement therapy, the American College of Nutrition recommends providing 100 mg thiamine daily due to increased losses 1
Prevention in High-Risk Groups
- The American College of Nutrition recommends that all bariatric surgery patients receive a complete multivitamin and mineral supplement containing at least the government-recommended daily allowance for thiamine 4, 6
- Patients should be educated about the risks of thiamine deficiency and instructed to seek early medical advice if they experience prolonged vomiting or poor dietary intake 4, 5
