Acidosis Treatment Guidelines

Understanding Acidosis Types

  • The treatment of metabolic acidosis focuses on addressing the underlying cause and may require bicarbonate supplementation when severe (pH < 7.2), while respiratory acidosis treatment primarily targets improving ventilation to reduce CO2 levels 1, 2
  • Metabolic acidosis causes include failure to excrete acids produced by normal metabolic processes (e.g., renal failure) and increased production of acids from abnormal metabolic conditions (e.g., diabetic ketoacidosis) 2
  • Respiratory acidosis is characterized by low blood pH (<7.35) with elevated CO2 levels, resulting from hypoventilation leading to CO2 retention 1
  • About 20% of patients with acute exacerbations of COPD have respiratory acidosis, with laboratory findings including elevated PaCO2, low pH, and normal or compensatory elevated bicarbonate 3

Treatment Approaches

  • In chronic kidney disease, pharmacological treatment may be considered when bicarbonate levels are less than 18 mmol/L, according to the American Journal of Kidney Diseases guidelines 4, 5
  • For lactic acidosis due to tissue hypoxia, the primary focus is on improving oxygen delivery 2
  • Mechanical ventilation may be required in acute respiratory acidosis to improve ventilation and reduce CO2 levels 3
  • Careful oxygen therapy is essential in chronic respiratory acidosis, as excessive oxygen can worsen hypercapnia in certain conditions like COPD 6
  • The American Journal of Kidney Diseases guidelines recommend avoiding excessive bicarbonate supplementation to prevent alkalosis 4
