DPP-4 Inhibitors for Ramadan Fasting Management

Introduction to DPP-4 Inhibitors

  • DPP-4 inhibitors are the safest oral glucose-lowering medication class for Muslim patients with type 2 diabetes fasting during Ramadan and typically require no dose adjustment 1, 2

Pre-Ramadan Preparation

  • Evaluate glycemic control, renal function, hepatic status, and complete biochemical profile before Ramadan 1
  • Risk stratify the patient using established Ramadan-specific diabetes risk categories 1, 4
  • Conduct trial fasting to test the current medication regimen before Ramadan begins 1
  • Provide structured education on hypoglycemia/hyperglycemia symptoms and when to break the fast 1, 4

Medication Management

  • Continue the same dose of DPP-4 inhibitors at the same time—no adjustment needed 1, 2
  • Adjust metformin timing: Give two-thirds of total daily dose immediately before sunset meal (Iftar) and one-third before predawn meal (Suhur) 6, 7, 5, 3
  • DPP-4 inhibitor dose remains unchanged 1, 2

Monitoring and Adjustments

  • Self-monitor blood glucose closely during the first 3-4 weeks, particularly in the first few hours after starting the fast and late afternoon before breaking fast 3
  • Break the fast immediately if blood glucose drops below 70 mg/dL 3
  • Monitor for complications and make further medication modifications as needed to prevent hypoglycemia or hyperglycemia 1, 4

Post-Ramadan Management

  • Assess overall health, complications, and glycemic control after Ramadan 1, 4
  • Return therapeutic regimen to pre-Ramadan schedule (dose and timing) 1, 4
  • Review the fasting experience to guide future Ramadan planning 1, 4
