Boswellia for Inflammatory Conditions

Efficacy and Recommendations

  • The American College of Rheumatology (ACR) and European League Against Rheumatism (EULAR) do not include Boswellia in their treatment recommendations for rheumatoid arthritis, with methotrexate remaining the anchor drug and NSAIDs like meloxicam used for symptom control 1, 2
  • The American Gastroenterological Association (AGA) recommends against Boswellia serrata treatment for microscopic colitis, citing concerns about standardization of formulations and uncertain benefit-risk balance, with low quality evidence 4, 5
  • For osteoarthritis, acetaminophen is recommended as first-line oral analgesic, and NSAIDs (oral or topical) for patients unresponsive to acetaminophen, according to the Annals of the Rheumatic Diseases 6
  • For rheumatoid arthritis and psoriatic arthritis, disease-modifying antirheumatic drugs (DMARDs) should be initiated rapidly, with methotrexate preferred, and NSAIDs used primarily for symptom control 7, 9
  • Boswellia should not be used as a replacement for established treatments for inflammatory arthritis conditions, according to Arthritis and Rheumatology and Annals of the Rheumatic Diseases 1, 7

Safety and Standardization

  • Standardized formulations of Boswellia should be used when available, as recommended by the American Gastroenterological Association (AGA) 4
