Administration of Morphine
Calculation and Preparation
- To determine the volume of morphine solution needed for a 2 mg dose, set up a proportion and solve for X, where 2 mg : X ml = 10 mg : 2 ml, yielding X = 0.4 ml 1, 2, 3
Administration Considerations
- For intravenous administration of morphine, administer slowly to minimize adverse effects, as recommended by the European Society for Medical Oncology 4
- For subcutaneous administration of morphine, use a small needle to minimize discomfort, according to the National Comprehensive Cancer Network 5
Clinical Guidelines
- The European Society for Medical Oncology recommends morphine as the opioid of choice for opioid-naïve patients requiring pain management 1, 2
- For intravenous titration in severe pain, the American Society of Clinical Oncology suggests 1.5 mg boluses every 10 minutes until pain relief is achieved 4
- The British Journal of Cancer reports that the subcutaneous route is an effective alternative if intravenous access is unavailable, with a relative potency ratio of oral to intravenous/subcutaneous morphine between 1:2 and 1:3 5
