Testosterone Gel Dosing Guidelines (Evidence‑Based)

Starting Dose and Formulation‑Specific Recommendations

  • Fortesta 2 % gel delivers 10 mg testosterone per pump actuation; the recommended initial regimen is 40 mg daily (four actuations) applied to the shoulders/upper arms. 1
  • Axiron 2 % solution delivers 30 mg testosterone per pump actuation; the recommended initial regimen is 60 mg daily (two actuations) applied to the axilla. 1
  • Typical daily dose ranges for testosterone gels (Gastroenterology 2017): 1 % gels = 50–100 mg, Fortesta 2 % gel = 10–70 mg. 1

Long‑Term Hormone Monitoring

  • After achieving stable serum levels, measure testosterone every 6–12 months to confirm continued adequacy. 4
  • The target serum testosterone is the mid‑normal physiological range, approximately 400–700 ng/dL, balancing efficacy and safety. 4
  • For transdermal gels, a blood sample may be drawn any time after ≥1 week of continuous daily use, reflecting steady‑state concentrations. 4

Hematocrit Surveillance

  • Check hematocrit at baseline, at 3–6 months, and annually thereafter. 4
  • Stop testosterone immediately if hematocrit exceeds 54 %, evaluate for hypoxia or sleep‑apnea, and consider re‑starting at a reduced dose only after normalization. 4

Prostate Monitoring (Men ≥ 40 years)

  • Obtain baseline PSA and digital rectal exam before initiating therapy. 4
  • Repeat PSA at 3–6 months and then annually. 4
  • Refer to urology if PSA rises > 1.4 ng/mL within 12 months, PSA velocity > 0.4 ng/mL / year, or the digital exam is abnormal. 4

Therapy Discontinuation Criteria

  • If serum testosterone reaches target levels but hypogonadal symptoms have not improved after 3–6 months, consider discontinuing testosterone gel. 5
