Lipid Panel Testing in Adults

Age-Specific Recommendations

  • The American Academy of Family Physicians recommends that routine lipid panel testing can be discontinued in adults over 75 years of age, unless the patient is on statin therapy or has specific cardiovascular risk factors that warrant continued monitoring 1, 2
  • For adults under 40 years not taking statins or other lipid-lowering therapy, the American Diabetes Association recommends obtaining a lipid profile at initial evaluation and every 5 years thereafter 3
  • The American Academy of Family Physicians suggests that more frequent testing is appropriate for those with diabetes, especially those with youth-onset type 1 diabetes 3
  • For men 20-35 years and women 20-45 years, the American Academy of Family Physicians recommends screening only if they have risk factors for coronary heart disease (diabetes, family history of premature CVD, hypertension, smoking) 2, 4
  • The American College of Cardiology strongly recommends lipid testing for all adults 40-75 years, including total cholesterol, LDL cholesterol, HDL cholesterol, and triglycerides 5, 6, 3
  • The American Academy of Family Physicians suggests a reasonable screening interval of every 5 years, with shorter intervals for those with lipid levels close to treatment thresholds 2, 1

Special Considerations for Older Adults

  • For adults over 75 years already on statin therapy, the American Diabetes Association recommends continuing monitoring to assess medication adherence and efficacy 3, 7
  • The American Academy of Family Physicians suggests that for adults over 75 years not on therapy, the predictive value of cholesterol levels for cardiovascular risk diminishes significantly after age 75 2, 1

Continuing Monitoring in Older Adults

  • The American Diabetes Association recommends continuing lipid monitoring in patients over 75 years who are already on statin therapy, have established atherosclerotic cardiovascular disease, have diabetes with multiple cardiovascular risk factors, or have recently had medication changes that might affect lipid levels 3

Discontinuing Lipid Testing

  • The American Academy of Family Physicians suggests considering discontinuing routine lipid testing in patients over 75 years who have consistently normal lipid levels and no cardiovascular disease 2, 1
