Tretinoin Treatment for Acne Vulgaris

Introduction to Tretinoin

  • The American Academy of Dermatology strongly recommends tretinoin as a first-line topical treatment for acne vulgaris, applied as a thin layer once daily before bedtime to affected areas 1, 2, 3

Mechanism and Efficacy

  • Tretinoin treats acne through multiple mechanisms: comedolytic, anti-inflammatory, and normalizing follicular keratinization 4, 5
  • In clinical trials, 21-23% of patients using topical tretinoin achieved successful treatment based on global severity scores 5

Dosing and Administration

  • Apply a thin layer of tretinoin once daily, before bedtime, to skin where acne lesions occur 5
  • Keep away from eyes, mouth, nasal creases, and mucous membranes 5

Formulations and Strengths

  • Tretinoin is available in multiple formulations: cream (0.025%, 0.05%, 0.1%), gel (0.01%, 0.025%), and microsphere gel 4
  • Lower concentrations (0.025%) may be better tolerated for patients with sensitive skin 4
  • Some formulations should be applied in the evening due to photolability and should not be used with benzoyl peroxide to avoid oxidation and inactivation 2

Treatment Approach Based on Acne Severity

  • For severe, scarring, or psychologically burdensome acne failing standard therapy: Consider oral isotretinoin 3

Side Effects and Management

  • Common side effects include dry skin, peeling, scaling, flaking, burning sensation, erythema, pruritus, and hyper/hypopigmentation 5

Important Considerations

  • Photosensitivity may occur; use daily sunscreen and minimize sun exposure after application 5, 4
  • Maintenance therapy with tretinoin should continue after acne clears to prevent recurrence 4

Combination Therapy

  • Multimodal therapy combining multiple mechanisms of action is recommended for optimal efficacy 3
  • Tretinoin can be effectively combined with benzoyl peroxide (not applied simultaneously with some tretinoin formulations) 2, 4
  • Tretinoin can be effectively combined with topical antibiotics (clindamycin, erythromycin) for inflammatory lesions 4
  • Fixed-dose combinations (clindamycin/tretinoin) can be used for patients ≥12 years of age 5

Common Pitfalls to Avoid

  • Using tretinoin as monotherapy for moderate-to-severe inflammatory acne 4
  • Failing to protect skin from sun exposure while using tretinoin 5, 4
