Ovarian Cancer Staging and Management
Surgical Staging
- Comprehensive surgical staging via median laparotomy performed by a trained gynecologic oncologist is required for all patients with suspected ovarian cancer, including peritoneal washings for cytological analysis, thorough abdominal cavity exploration, biopsy of diaphragmatic peritoneum, paracolic gutter biopsies, pelvic peritoneum biopsies, infracolic omentectomy, and systematic pelvic and para-aortic lymph node sampling or dissection 1, 2, 3, 4, 5
- The American College of Obstetricians and Gynecologists recommends that surgical staging should include a thorough inspection of the abdominal cavity, collection of peritoneal washings, and biopsy of any suspicious areas 1, 3, 4
Management by Stage
- For early-stage disease (FIGO Stage I-IIa), the standard surgical procedure includes total abdominal hysterectomy, bilateral salpingo-oophorectomy, omentectomy, random peritoneal biopsies, and at least pelvic/para-aortic lymph node sampling, with adjuvant chemotherapy recommended for stage Ia/Ib poorly differentiated, densely adherent, or clear cell histology AND all grades Stage Ic and IIA 1, 6, 7
- The National Comprehensive Cancer Network recommends carboplatin AUC 5-7 mg/ml/min + paclitaxel 175 mg/m² every 3 weeks for 3-6 cycles as the recommended regimen for adjuvant chemotherapy in early-stage ovarian cancer 1, 3
- For advanced disease (FIGO Stage IIb-IIIc), maximal upfront surgical cytoreduction with the goal of no residual disease is the standard approach, followed by 6 cycles of combination chemotherapy, with carboplatin AUC 5-7.5 mg/ml/min + paclitaxel 175 mg/m² over 3 hours every 3 weeks for 6 cycles as the standard chemotherapy regimen 1, 6, 7
Special Histologic Subtypes
- For high-grade endometrioid carcinoma, complete surgical resection including systematic pelvic and para-aortic lymph node dissection is standard, with adjuvant platinum-based chemotherapy recommended 5
- For clear cell carcinoma, adjuvant chemotherapy may be omitted for adequately staged IA or IB disease, but is recommended for stages IC2, IC3, and II 5
- For high-risk mucinous carcinoma, adjuvant platinum-based chemotherapy should be offered for stage I-II disease 5
