Management of Type 2 Diabetes Mellitus
Initial Assessment and Treatment Algorithm
- The American College of Physicians recommends starting metformin 500 mg daily alongside comprehensive lifestyle modifications immediately at diagnosis for all newly diagnosed T2DM patients, unless they present with ketosis, diabetic ketoacidosis, random blood glucose ≥250 mg/dL, or HbA1c ≥8.5%—in which case, initiate insulin therapy first 1, 2, 3
- Patients requiring immediate insulin therapy should start insulin immediately (0.5 units/kg/day) if ANY of the following are present: ketosis or diabetic ketoacidosis, random blood glucose ≥250 mg/dL, HbA1c ≥8.5%, or unclear distinction between Type 1 and Type 2 diabetes 1, 2, 3
Comprehensive Lifestyle Modifications
- The American Heart Association recommends implementing Mediterranean, DASH, or vegetarian/vegan dietary patterns focusing on fiber-rich whole grains, reduced red meat consumption, and complete elimination of sugar-added beverages 1, 2
- The Academy of Nutrition and Dietetics recommends referring to registered dietitian nutritionist with diabetes expertise at diagnosis, with nutrition plans being culturally appropriate and sensitive to family resources 1
- The American College of Sports Medicine recommends a minimum of 150 minutes per week of moderate-intensity OR 75 minutes per week of vigorous-intensity physical activity, combining aerobic and resistance training for adults 4, 1, 2
Monitoring Protocol
- The American Diabetes Association recommends checking HbA1c every 3 months until target achieved (<7% for most patients) 1, 2, 3
- Finger-stick blood glucose monitoring is indicated for patients taking insulin or medications with hypoglycemia risk, initiating or changing diabetes treatment regimen, patients not meeting treatment goals, or with intercurrent illnesses 1, 2, 3
Treatment Intensification Strategy
- The European Association for the Study of Diabetes recommends adding SGLT-2 inhibitor or GLP-1 receptor agonist for patients with additional ASCVD risk factors or established cardiovascular/kidney disease if glycemic targets are not achieved after 3 months on metformin 1, 2, 3
- GLP-1 receptor agonists provide 10-15% weight loss or more and are particularly effective for patients with obesity, but are contraindicated in patients with family history of medullary thyroid cancer 4, 1
Weight Management and Metabolic Surgery
- The International Diabetes Federation recommends targeting at least 5% weight loss for clinical benefits, with substantial weight loss (>10%) early in disease course increasing chance of remission 4
- Metabolic surgery should be considered for patients without sufficient response to non-surgical weight loss interventions, particularly early in disease course, and performed in high-volume centers with experienced multidisciplinary teams 4
Diabetes Self-Management Education
- The American Association of Diabetes Educators recommends providing comprehensive diabetes self-management education (DSMES) at diagnosis, annually, with changes in health status, and with transitions of care 4, 1
- Establishing SMART goals (Specific, Measurable, Attainable, Relevant, Time-based) for behavior change rather than non-specific recommendations is recommended 4
