Management of Molluscum Contagiosum in Children

Primary Treatment Recommendation

  • The American Academy of Ophthalmology recommends physical removal methods, such as curettage, simple excision, or excision with cautery, as first-line therapy for symptomatic, multiple, or periocular lesions in children 1

Treatment Algorithm

  • Watchful waiting is reasonable for asymptomatic lesions, limited number of lesions, and no periocular involvement, as lesions typically resolve spontaneously in 6-12 months 1
  • Active treatment is indicated for symptomatic lesions, multiple lesions, and lesions near the eyes causing conjunctivitis, to prevent autoinoculation and transmission 1

Treatment Modalities

  • Cryotherapy with liquid nitrogen achieves complete response in approximately 93% of cases, but has a risk of postinflammatory hyperpigmentation or scarring 1
  • The American Academy of Pediatrics recommends 10% potassium hydroxide solution as a first-line chemical treatment, with similar efficacy to cryotherapy and better cosmetic results 1

Special Considerations

  • Salicylic acid is contraindicated in children under 2 years due to risk of systemic toxicity 2
  • Periocular lesions with conjunctivitis require physical removal and monitoring for resolution of conjunctivitis at follow-up 1

Treatments to AVOID

  • Imiquimod is not effective and should not be used, as it showed no benefit compared to placebo in randomized controlled trials 1
  • Ranitidine or other H2 blockers have no evidence supporting efficacy for molluscum contagiosum and are not included in current guidelines from the American Academy of Pediatrics or American Academy of Dermatology 2

Prevention and Transmission Control

  • Hand hygiene is the most important method to prevent transmission, using alcohol-based disinfectant or soap and water 1
  • Avoid sharing towels, clothing, and personal items, and cover all lesions with waterproof bandages if water exposure is unavoidable 1
