Pseudomonas aeruginosa Treatment Guidelines
Introduction to Treatment Options
- The European Respiratory Society recommends ciprofloxacin, not levofloxacino, as the oral antibiotic of choice for Pseudomonas aeruginosa infections 1, 2
- Ciprofloxacin is recommended at high doses (750 mg every 12 hours) as the best available oral antipseudomonal antibiotic 1, 2, 3
Clinical Context for Treatment Choices
- For respiratory infections with risk of Pseudomonas, ciprofloxacin 750 mg every 12 hours is the preferred oral option 1, 2, 3
- For parenteral treatment, ciprofloxacin IV or antipseudomonal betalactams (ceftazidime, cefepime, piperacillin-tazobactam, meropenem) are recommended 1, 4, 3
- The Clinical Microbiology and Infection guidelines recommend ciprofloxacin (not levofloxacino) in combination with antipseudomonal betalactams for severe infections 4
Combination Therapy and Resistance
- For severe Pseudomonas infections, combination therapy with two antipseudomonal agents from different classes should always be used 4, 3, 5
- Monotherapy with fluoroquinolones should be avoided in severe infections due to the high risk of resistance development 5
- There is cross-resistance between levofloxacino and ciprofloxacin; strains resistant to ciprofloxacin are also resistant to levofloxacino 1, 2
Decision Algorithm for Antipseudomonal Coverage
- If oral coverage against Pseudomonas is needed, first-line treatment is ciprofloxacin 750 mg every 12 hours 1, 2, 3
- If parenteral coverage is needed, an antipseudomonal betalactam plus ciprofloxacin or an aminoglycoside is recommended 4, 3, 5
- For severe infections or ICU patients, combination therapy with two antipseudomonal agents should always be used, with adjustment according to susceptibility results after 48-72 hours 4, 3, 5
