Management of Restless Legs Syndrome and Fibromyalgia Pain

Diagnostic Workup for Restless Legs in Fibromyalgia

  • Measure serum ferritin and transferrin‑saturation in every patient with fibromyalgia who reports restless‑legs symptoms, as recommended by the American Academy of Sleep Medicine (strong recommendation). 1

Iron Repletion Thresholds

  • Initiate iron therapy when ferritin is ≤ 100 ng/mL or transferrin‑saturation is < 20 %; these cut‑offs are higher than those used for anemia treatment (strong recommendation, Praxis Medical Insights). 2
  • In patients with end‑stage renal disease, use a higher ferritin threshold (< 200 ng/mL) together with transferrin‑saturation < 20 % before giving intravenous iron sucrose (strong recommendation, Praxis Medical Insights). 3

Iron Repletion Strategies

  • Intravenous ferric carboxymaltate 1000 mg is the preferred first‑line iron formulation for patients meeting the above thresholds, providing rapid restoration of iron stores without gastrointestinal adverse effects (strong recommendation, Praxis Medical Insights). 2
  • Oral ferrous sulfate (325–650 mg daily or every other day) is an acceptable alternative when IV access is unavailable, though it works more slowly and commonly causes constipation (moderate recommendation, Praxis Medical Insights). 2

First‑Line Pharmacologic Treatment for Restless Legs

  • Start a gabapentinoid (gabapentin, gabapentin enacarbil, or pregabalin) immediately, irrespective of iron status (strong recommendation, American Academy of Sleep Medicine). 1
  • Gabapentin: begin 300 mg at bedtime; titrate to a total daily dose of 1800–2400 mg in divided doses (strong recommendation, American Academy of Sleep Medicine). 1
  • Gabapentin enacarbil: 600 mg taken once daily at 5 PM (strong recommendation, American Academy of Sleep Medicine). 1
  • Pregabalin: start 75 mg at bedtime; titrate to 150–450 mg per day (strong recommendation, American Academy of Sleep Medicine). 1
  • For patients on dialysis, use renally‑adjusted gabapentin dosing of 100–300 mg after each dialysis session, titrating every 3–7 days (moderate recommendation, Praxis Medical Insights). 3

Medications to Discontinue or Avoid

  • Stop agents that can exacerbate restless‑legs symptoms, including serotonergic antidepressants (SSRIs, SNRIs), dopamine antagonists (e.g., metoclopramide, prochlorperazine), and centrally acting antihistamines (e.g., diphenhydramine, hydroxyzine) (strong recommendation, Praxis Medical Insights). 2
  • Avoid dopamine agonists (pramipexole, ropinirole, rotigotine) as first‑line therapy because they carry a 7–10 % annual risk of augmentation, an iatrogenic worsening of symptoms that is difficult to reverse (conditional recommendation against use, American Academy of Sleep Medicine). 1

Fibromyalgia Pain Management

Exercise Therapy

  • Implement a graduated exercise program as the cornerstone of fibromyalgia treatment; exercise is the only intervention with a “strong for” recommendation based on high‑quality meta‑analyses (strong recommendation, Annals of the Rheumatic Diseases). 4

Pharmacologic Options for Severe Symptoms

  • When non‑pharmacologic measures are insufficient, consider duloxetine, milnacipran, pregabalin (which also treats RLS), or amitriptyline for severe pain or sleep disturbance (moderate recommendation, Annals of the Rheumatic Diseases).
  • Cyclobenzaprine may modestly improve sleep but is associated with side effects in ~85 % of patients; it receives a “weak for” recommendation (weak recommendation, Annals of the Rheumatic Diseases). 4

Expected Timeline of Therapeutic Effects

Intervention Expected Onset of Clinical Benefit Evidence Strength
Intravenous ferric carboxymaltate Noticeable improvement within weeks Strong (Praxis Medical Insights)
Oral ferrous sulfate Gradual improvement over 4–12 weeks Moderate (Praxis Medical Insights)
Gabapentinoids (therapeutic dose reached) Benefit typically seen within 1–2 weeks Strong (American Academy of Sleep Medicine)
Structured exercise program Meaningful pain reduction after 8–12 weeks Strong (Annals of the Rheumatic Diseases)
