Bacterial Vaginosis Treatment Guidelines

Key Recommendations

  • The Centers for Disease Control and Prevention (CDC) guidelines do not explicitly mandate sexual abstinence during BV treatment, but practical considerations strongly support this recommendation 1, 2
  • Patients should avoid sex until both they and their partners are cured (therapy completed and both are asymptomatic) 3
  • The CDC explicitly states that routine treatment of sex partners is NOT recommended 1, 2

Important Medication Considerations

  • Clindamycin cream is oil-based and can weaken latex condoms and diaphragms, making barrier contraception unreliable during treatment 2
  • Clindamycin cream creates an additional safety concern for sexual activity during the 7-day treatment course 2
  • Metronidazole requires avoiding all alcohol during treatment and for 24 hours after completion to prevent disulfiram-like reactions (flushing, nausea, vomiting, headache) 2

Common Pitfalls to Avoid

  • If using clindamycin cream, latex barrier methods may fail 2
  • Even small amounts of alcohol can trigger severe reactions with metronidazole—this includes alcohol in mouthwash or medications 2
  • Waiting until symptoms resolve is insufficient; complete the full treatment course first 3

Treatment Completion and Follow-Up

  • Follow-up visits are unnecessary if symptoms resolve 1, 2

Bacterial Vaginosis Treatment Guidelines

Introduction to Treatment

  • The Centers for Disease Control and Prevention recommends treating female patients with metronidazole 500 mg orally twice daily for 7 days, or alternative clindamycin 2% vaginal cream for 7 days if metronidazole is contraindicated 4, 5

Treatment for Male Partners

  • Male partners should be treated with metronidazole 400 mg orally twice daily for 7 days, plus 2% clindamycin cream applied topically to the penile skin twice daily for 7 days, to reduce BV recurrence 6

Critical Instructions for Patients

  • Patients should avoid alcohol during metronidazole treatment and for 24 hours after completion to prevent disulfiram-like reactions, as advised by the Centers for Disease Control and Prevention 4, 5, 6
  • Patients should refrain from unprotected intercourse for at least 14 days to allow treatment to take effect, although this is not directly cited, the importance of adherence and avoiding reinfection is emphasized 4

Adverse Effects and Follow-Up

  • Asymptomatic male partners may experience mild gastrointestinal upset, as noted by the Centers for Disease Control and Prevention 7
  • No routine follow-up is needed for asymptomatic male partners, according to the Centers for Disease Control and Prevention 7
