Postoperative Parathyroid Hormone Behavior

Changes in Postoperative PTH

  • The American Association of Endocrine Surgeons recommends that PTH levels decrease rapidly after total thyroidectomy due to damage or unintentional removal of the parathyroid glands during surgery 1, 2
  • The American Thyroid Association states that a PTH level >20 pg/ml measured 20 minutes after surgery does not require intensive calcium monitoring 1, 3

Technical Considerations in PTH Measurement

  • The Endocrine Society suggests that the same PTH assay should be used for all measurements in the same patient to ensure adequate evaluation of trends 1, 4
  • The use of third-generation assays may better reflect parathyroid function in the early postoperative hours, although specific comparative studies in thyroid surgery are lacking 1, 2
  • Clinical guidelines should avoid absolute cutoff values due to differences between various PTH assays 1, 5

Long-term Complications

  • The National Comprehensive Cancer Network reports that permanent hypoparathyroidism is a rare complication, occurring in approximately 2.6% of patients after total thyroidectomy 6
  • When surgery is performed by experienced surgeons, the rate of permanent hypoparathyroidism can be reduced to 0.5% 6
