Assessment and Management of Alcohol Withdrawal Seizure

Assessment Components

  • The American Association for the Study of Liver Diseases recommends documenting alcohol consumption pattern and duration to determine severity of alcohol use disorder (AUD) using DSM-V criteria 1
  • The assessment should include evaluating for comorbid medical conditions that may affect treatment approach, such as liver disease or other alcohol-related organ damage 1, 2
  • A neurological examination should be performed to rule out focal deficits that would suggest alternative etiology, and to assess for signs of head trauma that may have occurred during seizure 3, 4
  • The patient's vital signs should be monitored, with particular attention to tachycardia, hypertension, and hyperthermia as indicators of autonomic hyperactivity 2

Treatment Plan

  • The American College of Clinical Pharmacology recommends administering benzodiazepines as first-line therapy for seizure prevention and withdrawal symptom control, with dosing based on Clinical Institute Withdrawal Assessment for Alcohol (CIWA) scores 2, 5
  • Thiamine supplementation should be provided to prevent Wernicke encephalopathy, with a recommended dose of 100-300 mg/day 2, 5, 6
  • The patient should be monitored regularly for progression to more severe withdrawal symptoms, including hallucinations or delirium tremens 2

Disposition Planning

  • Inpatient admission should be considered for patients with a history of severe withdrawal symptoms or delirium tremens, multiple seizures or status epilepticus, or significant medical or psychiatric comorbidities 2, 5
  • The American Society of Addiction Medicine recommends arranging psychiatric consultation for evaluation, treatment, and long-term planning for alcohol abstinence 2
  • Education should be provided about the risks of continued alcohol use and the benefits of abstinence, and the patient should be connected with addiction treatment resources 1
