Management of Established Tetanus

Immediate Priorities in Established Tetanus

  • The American College of Physicians recommends securing the airway immediately and preparing for mechanical ventilation, as respiratory failure from laryngospasm and respiratory muscle rigidity is the leading cause of death in tetanus 1
  • The case fatality rate remains 18-21% even with modern intensive care, primarily due to respiratory complications and autonomic dysfunction 2

Wound Management

  • The Infectious Diseases Society of America recommends performing thorough surgical debridement of all necrotic tissue to remove anaerobic conditions favorable for Clostridium tetani growth 1, 2
  • Proper wound cleaning and debridement are as critical as immunization in tetanus management 2

Immunologic Treatment

  • The Centers for Disease Control and Prevention recommends administering human TIG 250 units IM immediately for prophylactic dosing in established tetanus 3, 1, 2
  • The Advisory Committee on Immunization Practices recommends using only adsorbed toxoid when TIG and tetanus toxoid are given together 3
  • The standard of care remains IM administration of TIG 3, 1, 2

Active Immunization

  • The American Academy of Pediatrics recommends administering tetanus toxoid vaccine immediately during acute management, as tetanus infection does not confer immunity 1
  • The Centers for Disease Control and Prevention recommends using Td (tetanus-diphtheria toxoid) as the preferred preparation for adults ≥7 years of age 2

Special Population Considerations

  • The American Geriatrics Society notes that older adults have significantly higher mortality from tetanus 2
  • The Infectious Diseases Society of America prioritizes TIG administration in patients >60 years if supplies are limited, as they are less likely to have protective antibody levels 1, 2
  • The Centers for Disease Control and Prevention recommends that severely immunocompromised patients require TIG regardless of vaccination history 1

Tetanus Vaccination in Acute Management

Critical Immunologic Principles

  • Natural tetanus infection provides no immunity whatsoever—patients who survive tetanus remain fully susceptible to future infections unless actively immunized with tetanus toxoid, as recommended by the Centers for Disease Control and Prevention 4

Immediate Treatment Protocol for Established Tetanus

Active Immunization

  • The American Academy of Family Physicians recommends administering tetanus toxoid vaccine (Td preferred for adults ≥7 years) at the same time as TIG, but at a separate anatomic site using a separate syringe, to initiate active immunity that will protect the patient after recovery 4
