Acid Suppression Therapy

Pharmacokinetics and Administration

  • Pantoprazole requires 3-5 days to reach maximal acid suppression and should be taken 30-60 minutes before meals for optimal effect, with a half-life of 6-9 hours 1
  • Pantoprazole is affected by CYP2C19 genetic polymorphisms, which can lead to variability in therapeutic outcomes 3

Clinical Efficacy and Applications

  • The American Gastroenterological Association recommends pantoprazole for moderate to severe GERD, erosive esophagitis, or peptic ulcer disease, as it provides more consistent and profound acid suppression in these conditions 2
  • Famotidine may be appropriate as first-line therapy for mild, intermittent symptoms, but its efficacy may diminish with continued use due to tachyphylaxis (tolerance) 2

Common Pitfalls and Caveats

  • Acid rebound can occur upon discontinuation of both medications, but this effect is typically more pronounced with PPIs like pantoprazole 4
  • Cost considerations suggest that famotidine is generally less expensive and more readily available over-the-counter than pantoprazole 5
