Lactulose Use in Special Populations

Clinical Considerations

  • The American Association for the Study of Liver Diseases suggests that lactose itself can be used as an alternative to lactulose for hepatic encephalopathy treatment in populations with high lactose intolerance prevalence 1
  • The residual lactose content in lactulose (less than 1.2 g per dose) is substantially lower than the typical lactose load that triggers symptoms in lactose-intolerant patients, with a moderate level of evidence supporting its safety in this population 2
  • For diabetic patients, careful carbohydrate monitoring is required when prescribing lactulose, due to its galactose and lactose content, with a moderate level of evidence supporting this recommendation 1
  • Initial dosing of lactulose for hepatic encephalopathy is 25 mL every 1-2 hours until producing 2-3 soft stools daily, with a high level of evidence supporting this dosing regimen 1, 3

Dosing Guidelines

  • Standard dosing ranges from 10-20 g (15-30 mL) daily up to 40 g (60 mL) daily for constipation, with a moderate level of evidence supporting this dosing range 2
