Magnesium Monitoring and Correction in Atrial Fibrillation Management

Pre‑initiation Monitoring for QT‑Prolonging Antiarrhythmics

The American College of Cardiology (ACC) recommends that serum magnesium and potassium levels be measured and corrected before starting QT‑prolonging antiarrhythmic drugs such as ibutilide, dofetilide, or sotalol to reduce the risk of torsades de pointes 1.

Electrolyte Optimization Prior to Pharmacological Cardioversion

The ACC advises correction of hypomagnesemia and hypokalemia before attempting pharmacological cardioversion in atrial fibrillation patients 1.

Ongoing Magnesium Surveillance During Antiarrhythmic Therapy

The ACC suggests repeat magnesium level assessments every 3–6 months for individuals receiving QT‑prolonging antiarrhythmic medications 1.
