Cross-Tapering from Abilify to Seroquel

Rationale and Protocol

  • The American College of Psychiatry recommends gradual cross-titration when switching antipsychotics to minimize withdrawal symptoms and maintain symptom control, considering the half-life and receptor profile of each medication 1
  • Aripiprazole (Abilify) has a long half-life of approximately 75 hours, while quetiapine (Seroquel) has a much shorter half-life of 6-7 hours, requiring different tapering considerations 2

Dosing and Monitoring

  • Start Seroquel 25mg at bedtime and reduce Abilify from 10mg to 7.5mg after 3-4 days, monitoring for sedation, orthostatic hypotension, and dizziness 1, 2
  • Increase Seroquel to 50mg at bedtime and reduce Abilify from 7.5mg to 5mg, continuing to monitor for extrapyramidal symptoms (EPS) and metabolic side effects 1, 2
  • Obtain baseline weight, BMI, fasting glucose, and lipid panel before starting Seroquel, and monitor weight weekly during the cross-taper and monthly thereafter 2

Critical Monitoring Parameters

  • Watch for aripiprazole discontinuation symptoms, including headache, agitation, anxiety, insomnia, dizziness, and drowsiness, which typically emerge within 1-4 days of dose reduction 2
  • Monitor blood pressure at each visit during titration, and be aware of quetiapine-specific adverse effects, including sedation, orthostatic hypotension, and dizziness 2

Important Clinical Considerations

  • Use lower starting doses in younger patients, such as the 25mg starting dose for this 13-year-old patient, and titrate more slowly than in adults to assess tolerability 1, 2
  • Use immediate-release quetiapine tablets only, allowing for flexible dosing during cross-taper, and never crush extended-release formulations to avoid dose dumping 2, 3
  • Continue to monitor for potential additive effects on heart rate and blood pressure from both stimulant and antipsychotic medications 2
