Management of Port-a-Cath Removal in Catheter-Related Bloodstream Infection
Immediate Removal Indications
- The Infectious Diseases Society of America recommends immediate removal of port-a-cath for complicated infections, including port pocket abscess or tunnel infection, which requires removal plus 7-10 days of antibiotics and incision/drainage if indicated 2, 3
- Immediate removal is required for S. aureus bacteremia, which has a high risk of metastatic complications, including endocarditis, and requires transesophageal echocardiography to rule out vegetations 1, 2
- The Infectious Diseases Society of America recommends removal of the catheter and treatment with antifungals for 14 days after the last positive blood culture for Candida species 1, 4
- Persistent bacteremia ≥72 hours despite appropriate antibiotics indicates complicated infection requiring catheter removal 1, 5
- Septic thrombosis or endocarditis requires removal and 4-6 weeks of antibiotics 1, 6
Catheter Salvage Approach
- The Infectious Diseases Society of America recommends attempting salvage only for uncomplicated infections with coagulase-negative staphylococci in hemodynamically stable patients without tunnel/pocket infection 1, 7
- Combine systemic antibiotics with antibiotic lock therapy for 2 weeks, using high-concentration antibiotics instilled into the catheter lumen with 12-24 hour dwell time 1, 7
Timing of Port Reinsertion After Removal
- Start appropriate systemic antibiotics based on organism susceptibilities, obtain negative repeat blood cultures after catheter removal, and complete the full antibiotic course before reinserting a new port 7, 6
- Wait an additional 5-10 days after completing antibiotics, then obtain surveillance blood cultures before proceeding with new port placement 7, 6
- For S. aureus with endocarditis, complete 4-6 weeks of antibiotics, then wait 5-10 days with negative cultures before reinserting a new port 6
- For uncomplicated coagulase-negative staphylococci, wait 5-7 days after removal before reinserting a new port 4
- For Candida species, wait 14 days after the last positive blood culture before reinserting a new port 4
