Metformin Management Around Iodinated Contrast Imaging

Risk Stratification and Management

  • The European Society of Cardiology guidelines suggest that there is "no convincing evidence" for routine metformin suspension before angiography, and propose checking renal function after the procedure and withholding metformin only if function deteriorates as an acceptable alternative 2
  • For patients with eGFR 30-60 mL/min/1.73 m², the KDIGO guidelines recommend withdrawal of potentially nephrotoxic agents, including metformin, before contrast procedures, and ensuring adequate saline hydration before, during, and after the procedure to minimize contrast-induced nephropathy risk 1, 3
  • In patients with normal renal function (eGFR ≥60 mL/min/1.73 m²) undergoing intra-arterial contrast administration, metformin should be stopped due to the higher nephrotoxicity risk associated with this route of administration 1
  • The KDIGO guidelines recommend using eGFR calculation rather than serum creatinine thresholds alone to assess renal function in patients undergoing contrast procedures 1, 3, 4

Special Considerations

  • For patients with eGFR 30-60 mL/min/1.73 m² requiring colonoscopy with bowel preparation, metformin should be discontinued before the procedure, and phosphate-containing bowel preparations should be avoided in favor of PEG-based alternatives 1, 3, 5
