Gabapentin Tapering Guidelines

Standard Tapering Protocol

  • The recommended approach for tapering gabapentin is to reduce the dose by 10-25% every 1-7 days to minimize withdrawal symptoms, and to monitor for withdrawal symptoms after each dose reduction before proceeding to the next step, as suggested by Praxis Medical Insights 1

Tapering Strategy Based on Daily Dose

  • For patients on 1800 mg/day or less, the Mayo Clinic recommends reducing the dose by 300 mg every 3-7 days, with an example taper being 1800 mg → 1500 mg → 1200 mg → 900 mg → 600 mg → 300 mg → discontinue 2
  • For patients on doses higher than 1800 mg/day, the Mayo Clinic suggests an initial reduction to 1800 mg/day, then following the standard tapering protocol 2

Managing Withdrawal Symptoms

  • If withdrawal symptoms occur, Praxis Medical Insights recommends returning to the previous well-tolerated dose 1
