Perioperative Management of Clopidogrel

General Recommendations for Clopidogrel Discontinuation

  • The American College of Cardiology/American Heart Association guidelines recommend discontinuing clopidogrel 5 days before elective surgery to minimize bleeding risk while allowing adequate time for platelet function recovery 1, 2
  • Clopidogrel should be discontinued 5 days before elective surgery according to the American College of Cardiology/American Heart Association guidelines 2
  • Discontinuation timing should be based on the type of P2Y12 inhibitor, with clopidogrel recommended to be discontinued 5 days before surgery, ticagrelor 3-5 days before surgery, and prasugrel 7 days before surgery 1, 2, 3

Surgery-Specific Considerations

  • The American College of Chest Physicians recommends interruption of P2Y12 inhibitors over continuation perioperatively for patients undergoing CABG surgery specifically 1, 3
  • For urgent CABG, clopidogrel should be discontinued for at least 24 hours before surgery to reduce major bleeding complications 2, 4

Risk Stratification Approach

  • For patients with coronary stents placed within the last 6-12 weeks, consider either continuation of dual antiplatelet therapy or stopping only one agent, as recommended by the American College of Chest Physicians 1, 3
  • For drug-eluting stents, elective surgery should ideally be postponed for at least 6 months after placement, according to the American College of Cardiology 5

Resumption of Antiplatelet Therapy

  • Antiplatelet drugs should be resumed within 24 hours after surgery rather than later to reduce thrombotic risk, as recommended by the American College of Chest Physicians 1, 3
  • For CABG specifically, P2Y12 inhibitors should be resumed within 24 hours after surgery, according to the American College of Chest Physicians 1

Special Considerations

  • Routine use of platelet function testing prior to surgery is not recommended to guide perioperative antiplatelet management, as stated by the American College of Chest Physicians 1, 3

Perioperative Management of Clopidogrel Before TURP

Platelet Function Recovery

  • For each day after interruption of clopidogrel, approximately 10-14% of normal platelet function is restored 6
  • When resuming at the maintenance dose (75 mg/day), it takes 5-10 days to attain maximal platelet inhibition 6
  • A loading dose (300-600 mg) can achieve maximal platelet inhibition within 12-15 hours if rapid antiplatelet effect is needed 6
