Combining Paracetamol with Ketoprofen for Pain Management

Guideline Recommendations

  • The PROSPECT guidelines strongly recommend (Grade A) combining paracetamol with NSAIDs or COX-2 inhibitors as basic postoperative analgesia for patients undergoing total hip arthroplasty 1, 2, 3
  • The ERAS Society guidelines recommend multimodal opioid-sparing analgesia using a combination of paracetamol and NSAIDs given orally for patients undergoing surgery 4
  • Guidelines for total knee arthroplasty recommend NSAIDs in combination with paracetamol (Grade D) for postoperative pain management 5, 6

Safety and Efficacy

  • The American College of Surgeons and other guideline societies imply that combining paracetamol with NSAIDs is safe and effective for pain management, as long as maximum daily doses are not exceeded and patient risk factors are assessed 1, 2, 4, 5, 6

Dosing Recommendations

  • The maximum daily dose of paracetamol should not exceed 4000 mg/day, with lower doses recommended for patients with liver disease 7
  • Ketoprofen plaster should be used according to the manufacturer's instructions, with consideration of total NSAID exposure if the patient is taking other NSAIDs systemically 7

Patient Assessment and Monitoring

  • Patients should be assessed for contraindications to paracetamol and ketoprofen, including severe hepatic impairment, active peptic ulcer, severe renal impairment, cardiovascular disease, and aspirin-sensitive asthma 5, 6, 7
  • Patients should be monitored for adequate pain control, local skin reactions, and signs of systemic NSAID effects 2, 4, 6
