Off-Label Uses for Lamotrigine
Neuropathic Pain Management
- The American Society of Clinical Oncology does not recommend lamotrigine for chemotherapy-induced peripheral neuropathy treatment, based on a randomized controlled trial showing no difference compared to placebo, with a higher dropout rate in the lamotrigine arm 1
- Other anticonvulsants like gabapentin (>1200 mg/day) and pregabalin (100-300 mg/day) are preferred options for neuropathic pain in cancer patients, according to the American Society of Clinical Oncology 1
- The American Gastroenterological Association suggests that gabapentin and pregabalin are options for visceral pain in gastroparesis, but lamotrigine is not recommended due to limited evidence 2, 3, 4
Psychiatric Indications
- The American College of Psychiatry recommends lamotrigine for maintenance treatment in adults with bipolar disorder to prevent new manic and/or depressive episodes, with maintenance recommended for at least 2 years after the last episode 5
- The American Academy of Child and Adolescent Psychiatry suggests that lamotrigine is commonly used off-label in pediatric populations (≥12 years) for bipolar disorder, despite lithium being the only FDA-approved agent for this age group 6
Important Clinical Considerations
- The American College of Clinical Pharmacy recommends slow titration of lamotrigine to minimize risk of serious rash, including Stevens-Johnson syndrome, with a standard titration period of 6 weeks to reach 200 mg/day 5
- The American College of Clinical Pharmacy notes that valproic acid significantly increases lamotrigine half-life, requiring substantial dose reduction, and that enzyme inducers may require higher lamotrigine doses 5, 7
- The American College of Clinical Pharmacy advises that combined hormonal contraceptives may reduce lamotrigine effectiveness, requiring monitoring and potential dose adjustment 5, 8
- The American College of Clinical Pharmacy states that a history of previous rash with lamotrigine is an absolute contraindication for reinitiation 7
