Anticoagulation for Genitourinary Tract Malignancy
Initial Treatment
- The American Society of Clinical Oncology recommends low-molecular-weight heparin (LMWH) as the preferred anticoagulant for patients with genitourinary malignancies and venous thromboembolism (VTE) due to the significantly increased risk of major bleeding with direct oral anticoagulants (DOACs) in this population 1
- LMWH is strongly recommended for initial anticoagulation when creatinine clearance is ≥30 mL/min, with options including dalteparin, enoxaparin, and tinzaparin 1, 2, 3, 4, 5
Long-Term Treatment
- The American Society of Clinical Oncology recommends LMWH for at least 6 months as the preferred long-term treatment for genitourinary malignancies, reducing VTE recurrence by 42% compared to vitamin K antagonists (VKAs) 1, 3, 4
- LMWH monotherapy is preferred, with dalteparin and enoxaparin as options, and the dose adjusted based on patient weight and renal function 1, 3, 4, 5
Duration of Anticoagulation
- A minimum of 6 months of anticoagulation is required, with continuation indefinitely for patients with metastatic disease or receiving active chemotherapy 1, 3, 4
- The risk-benefit ratio should be reassessed every 3-6 months for patients on extended therapy 1, 6
Special Considerations
- The American Society of Clinical Oncology recommends avoiding LMWH if creatinine clearance <30 mL/min, and considering unfractionated heparin (UFH) or dose-adjusted fondaparinux in severe renal impairment 1, 3, 4
- For patients with thrombocytopenia, monitor platelet counts every 2-3 days for the first 2 weeks on heparin products, and avoid anticoagulation if platelet count <50,000/μL 6, 7
Surgical Prophylaxis
- The American College of Chest Physicians recommends starting pharmacologic thromboprophylaxis preoperatively with UFH or LMWH for patients undergoing major surgery, and continuing for at least 7-10 days postoperatively 1, 3, 4
- Extend prophylaxis to 4 weeks for major abdominal or pelvic surgery with high-risk features (restricted mobility, obesity, prior VTE) 1, 3, 4
